Campbellford Memorial Hospital (CMH) has launched a new initiative aimed at improving patient care and reducing hospital readmissions in rural communities. The Rural Stop Gap (RSG) Program, introduced in partnership with Gotcare, leverages technology to provide support for patients recovering at home.
This pilot program addresses challenges faced by rural patients, including long wait times and limited resources, by offering a hybrid care model. It combines in-person visits from Health Ambassadors with virtual consultations from clinicians, ensuring patients receive timely, comprehensive care post-discharge.
The program, which initially served the Quinte area, has expanded to include all patients of CMH. It offers services such as remote monitoring, falls management, wellness check-ins, and personalized care plans. The use of AI technology allows for proactive health management by identifying potential issues before they become critical.
The RSG Program is part of CMH’s broader strategy to enhance healthcare access in rural areas and improve patient outcomes. By providing structured support at home, the program helps patients recover more effectively while easing hospital congestion.
